Before meeting with a funeral representative, gather the following documents and details, where applicable. 

Details

• Full name, birthdate and home address
• Parents’ names and birthplaces, including mother’s maiden name
• Copy of the deceased’s will
• Executor’s contact details
• Next of kin’s contacts
• Deceased’s place of birth
• Deceased’s occupation
• Each parent’s place of birth
• Whether the deceased had a pacemaker, defibrillator and/or radioactive implants (date of implant and last radioactive treatment)

• Citizenship and passport
• Social Insurance Number
• Birth Certificate
