Mount Pleasant Group (MPG) is proud to recognize two outstanding hospice organizations whose work is strengthening hospice palliative care through innovation and equity. Each recipient of this year’s MPG Hospice Awards receives $10,000 in support of their continued impact. The winners will be announced and presented at the HPCO Annual Conference in mid-June. 

The MPG Hospice Innovation Award, founded in 2019, was awarded to Hospice Georgian Triangle for its Palliative Care Resource Nurse (PCRN) role. This is an innovative partnership that embeds hospicefunded palliative expertise within acute care at Collingwood General and Marine Hospital. By supporting earlier access to a palliative approach, strengthening care transitions and mentoring healthcare teams, the program has improved patient and family experiences while building lasting capacity within the healthcare system.

The MPG Equity in Action Award, first awarded in 2025, was presented to Hospice Niagara for Beyond Hospice Walls, a communityembedded model that expands access to hospice palliative care for equitydeserving populations including 2SLGBTQIA+ and Indigenous communities. By delivering hospice, grief and wellness supports through trusted community partners, the initiative helps reduce barriers related to stigma, access and cultural safety, reaching people where they feel most comfortable and supported.

Together, these award‑winning initiatives reflect the evolving role of hospice care in Ontario and the power of thoughtful, community‑centred approaches to care at the end of life.

By recognizing and investing in innovative and equitable approaches to care, MPG is honoured to help strengthen hospice palliative care for individuals and families today and for generations to come.
